South Hadley, MA: On or about September 6, 2024, an employee was exposed to fall hazards of 11 feet 7 inches above a lower level while performing residential construction work and was not protected from falling by guard rail systems, safety net systems or a personal fall arrest system. LCT Construction & Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1523158, citation number 2, item 1, and was affirmed as a final order on 9/1/2021, with respect to a workplace located at 40 Blacksmith Rd., Wilbraham, MA.
