29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Location: Facility The company's lock out / tag out ( LOTO ) procedures are inadequate. Instance A: Written LOTO procedures were not developed for several machines. Instance B: Employees do not lock out equipment when performing some maintenance and servicing tasks as called for in the written procedures. Instance C: Written LOTO procedures lack details that would let workers know how to successfully de-energize equipment. Instance D: Written LOTO procedures are not in Spanish, the primary language for most maintenance employees. The Channel Fish Processing Co., Inc. was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1466770, citation number 1, item number 1, and was affirmed as a final order on March 30, 2020, with respect to a workplace located at 200 Commerce Drive in Braintree, MA.
