29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) 3008 Penns Valley Pike, Centre Hall, PA 16828 - On or about September 2, 2023, an employee injured their left knee on a piece of equipment after falling from a catwalk resulting in work restrictions. The employer did not record the workplace injury on the OSHA Form 300 or equivalent for the calendar year 2023 provided on October 11, 2023. b) 3008 Penns Valley Pike, Centre Hall, PA 16828 - On or about October 13, 2023, an employee was injured when a pallet was lowered onto the employee's left foot resulting in days away from work. The employer did not record the workplace injury on the OSHA Form 300 or equivalent for the calendar year 2023 provided on January 29, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
