29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about August 31, 2023, the employer failed to obtain manufacturer approval for the Tygard Claw attachment(s) prior to placing into service. According to the Tygard Claw's name plate the rated capacity was 2,000lbs lowering the forklift's 24in center load capacity by approximately 1,200lbs. b) On or about August 31, 2023, the employer failed to obtain manufacturer approval for the fork extender attachment prior to placing into service. The forklift extension's rating guide indicate that a 24in truck rated capacity at 24in load center of 4,000lbs is reduced to 1,900lbs at a 48in load center.
