On or about August 16, 2023, the employer did not initiate and maintain an effective safety and health program to ensure hazard detection and correction of hazards while employees installed framing lumber on the garage roof of a new build residential home. The employees were exposed to a fall distance of 10 feet 4 inches to the ground level as a result. EVT Building LLC was previously cited for a repeat serious violation of this occupational safety and health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1572692, citation number 2, item number 1a and was affirmed as a final order on 30 JUNE 2022, with respect to a workplace located at Vicinity Of Lot 138, 5518 Oakhurst Drive, Hebron, OH 43025. EVT Building LLC was previously cited for a repeat serious violation of this occupational safety and health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1559472, citation number 2, item number 1 and was affirmed as a final order on 02 DECEMBER 2021, with respect to a workplace located at 3779 Mystic Way, Lot 134, Grove City, Ohio 43123. EVT Building LLC was previously cited for a serious violation of this occupational safety and health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1475141, citation number 1, item number 1 and was affirmed as a final order on 24 JUNE 2020, with respect to a workplace located at 5203 Meadowsview Ln, Lot #50, Hilliard, Ohio 43026.
