29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Jobsite located at 1222 N 2nd Street, Philadelphia, PA 19122- On or about June 16, 2023, the employer did not ensure that each employee in an excavation was protected from cave-ins by an adequate protective system. Employees were performing excavation to perform underground utility work within the excavation measuring approximately 6 feet deep without the use of shoring or other protective system, thereby exposing employees to cave-in hazards.
