Text For Citation: 02 Item/Group: 001A Hazard:
29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (a) On or about February 21, 2023, at an establishment located at 1097 Ehlers Street, in Neenah, Wisconsin; in the Display Area for the FMC 3001 Flexo Press, S/N: 3833, the employer did not document each of the specific procedural steps for de-energization when performing cleaning activities to include but not limit the anilox roller, print roller/plate, and carry drum exposing employees to caught between hazards. (b) On or about February 21, 2023, at an establishment located at 1097 Ehlers Street, in Neenah, Wisconsin; in the Display Area for the FMC 3001 Flexo Press, S/N: 3833, the employer did not document each of the specific procedural steps for de-energization when performing set up activities exposing employees to caught between hazards. The Dixon Ticonderoga Company was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 1580960, citation number 2 item number 1 and was affirmed as a final order on June 27, 2022, with respect to a workplace located at 1097 Ehlers Road, in Neenah, Wisconsin.
