29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about February 9, 2023, the employer did not ensure that employees utilized lockout/tagout procedures for Core Mold Machine #U150 when making adjustments to the machine's pneumatic and flame pressures. Employees are exposed to struck by and crushing injuries during rotation/movement of the machine.
