29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. Paredes Inc. dba Superior Service located at 75 Garrett Trail, Maxwell, TX 78656. On or about November 30 , 2021, an employee died as a result of being caught under a section of a double wide mobile home. The fatality was not reported to the Austin Area Office of the Occupational Safety and Health Administration within eight (8) hours.
