29 CFR 1918.11(a)(1):The employer did not have annual examinations of gear certification under ILO 152: (a) on or about 9/22/2021 and at times prior thereto, employees working to unload the f/v Dominic II were exposed to struck-by hazards of falling objects, in that the safety and integrity of the ship mounted hoist was not ascertained and documented by annual examinations. (b) on or about 9/22/2021 and at times prior thereto, employees working to unload the f/v Silver Point were exposed to struck-by hazards of falling objects, in that the safety and integrity of the ship mounted hoist was not ascertained and documented by annual examinations. Pacific Seafood was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1917.50(a), which was contained in OSHA inspection number 1214177, citation number 1, item number 1, and was affirmed as a final order on 4/4/2017, with respect to a workplace located at 213 S.W. Bay Blvd. Newport, OR 97365.
