29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Instance a - 2 Northeast Blvd, Sterling, MA: On and before September 23, 2021 and continuing through at least March 1, 2022, the employer had not developed, documented, and utilized energy control procedures for employees performing screen changes on extruder line 16, to protect employees from uncontrolled thermal energy hazards. On September 23, 2021, an employee was severely burned while performing a screen change on extruder line 16 without all energy sources being isolated and controlled. Instance b - 2 Northeast Blvd, Sterling, MA: On and before February 3, 2022 and continuing through at least March 1, 2022, the employer had not developed, documented, and utilized energy control procedures for employees performing screen changes on extruder line 16, to protect employees from the electrical and potential hydraulic (hydraulic accumulator) energy sources.
