29 CFR 1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident: At rear of warehouse located at 1496 Mt. Zion Road, Morrow, GA 30260: On or about 7/20/2021, the employer failed to report a fatality within 8 hours after learning an employee had died after being hospitalized for injuries suffered in a scaffold incident on 07/19/2021. The employer did not report the fatality until 07/23/2021 at 4:15 PM.
