29 CFR 1910.147(c)(6)(i)(B):The periodic inspection shall be conducted to correct any deviations or inadequacies identified. a. On or about and prior to February 3, 2021, the employer failed to conduct a periodic inspection of the for the Line 7, tab tape area and the threading of the tab tape. Employees regularly used a bypass key to perform this task and enter the danger zone inside the CATS taper door in the tape tab area of Line 7. The employer's periodic inspection failed to include this task, identify any deficiencies and correct deviations in the energy control procedure. On January 28, 2021, an employee suffered an amputation while cleaning the knife and anvil using a bypass key to open the door and perform the task without shutting down the equipment. Associated Hygiene Products, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i), which was contained in OSHA inspection number 1244923, citation number 1, item number 1a and was affirmed as a final order on October 27, 2017, with respect to a workplace located at 2332 US Highway 42 South, Delaware, OH 43015.
