29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: On or about November 27, 2018: a) 4-Mill #3 - employees performing tasks including to but not limited to walking, setting up, maintenance, troubleshooting, removing jams, and minor tool changes were exposed to fall hazards where the employer did not cover or otherwise guard pits. Employees were exposed to falling approximately 12-inches inside the pit located in the walking/working surface to the side and front the machine. Kumho Tire Georgia, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1910.22(c), which was contained in OSHA inspection number 1196140, citation 1, item 2 issued June 6, 2017 and was affirmed as a final order on June 30, 2017, with respect to a workplace located at 3051 Kumho Parkway, Macon GA 31216.
