29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) North building - On or before November 28, 2018 the employer did not develop, document and utilize procedures for energy control exposing employees to amputation hazards from energy sources such as but not limited to electrical and kinetic energy while performing tasks including but not limited to start-up, maintenance, servicing, and jam clearing of the Farrell Compound Mixer.
