29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. A work related incident which resulted in a fatality was not reported to OSHA within 8 hours. The victim died on or about 11/15/2018 at 2:44 pm and was reported to OSHA on 11/19/2018 at 1:55 pm.
