29 CFR 1904.7(b)(2): An injury or illness that resulted in death was not recorded by entering a check mark on the OSHA 300 Log in the space for cases resulting in death. Rockbranch Ironworks, Coulterville, IL: On or about 09/20/2018, the employer did not record a death, on the OSHA 300 log for the calendar year of 2018. On or about 09/20/2018, a fatality occurred when an employee was electrocuted while performing welding operations at a jobsite. The death was not recorded on the OSHA 300 log.
