29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a) On or about June 28, 2018 at 20400 Powell Road, Brooksville, FL, an employee suffered a work related injury, was hospitalized and died in the hospital at 11:50 am on June 29, 2018. The employer was aware that same day that the employee had passed. The employer notified OSHA on July 3, 2018 at 12:00 P.M. of the employee's death.
