29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation: On or about March 26, 2018, an OSHA Form 300 or equivalent for the following work-related injuries or illnesses was not completed in the detail required. a. Loader working on press 4 was injured on 03/12/2018 and suffered injuries that required days away from work. The OSHA 300 log did not include the injuries suffered by the employee or an estimated number of days away from work. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
