Text For Citation: 02 Item/Group: 001A Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about January 11, 2018, line #1 employees were exposed to caught-in hazards while performing servicing activities at a dump auger hopper. The employees were not required to control hazardous energy in the following sequence prior to starting work: (a) prepare for shut down by recognizing energy hazards and its controls, (b) turn off and shut down the machines, (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (d) affix lockout or tagout devices to each energy isolating device, (e) render safe all potentially hazardous stored or residual energy, (f) verify that isolation and de-energization of the machine or equipment have been accomplished. Employee(s) performing servicing tasks had their hands/fingers exposed to caught-in hazards when removing guarding to perform servicing tasks including magnet checks and removal of residual food product from around the rotating auger. Employees were exposed to caught-in hazards likely to result in an amputation. The employer Crest Foods Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1133012, citation number 1, item number 3 and was affirmed as a final order on August 18, 2016, with respect to a workplace located at 502 Brown Avenue, Ashton, Illinois. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
