29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces. a. Wilbert, Inc. located in Bellevue, Ohio: On or about October 30, 2017, the employer did not ensure a maintenance employee repairing Press A5 was provided with a safe means of access and egress to the top of the press when repairing a hydraulic seal. While accessing the top of the equipment by climbing on the exterior of the frame, the employee was exposed to slip and fall hazards due to oil on the frame and uneven hand and footholds. b. Wilbert, Inc. located in Bellevue, Ohio: On or about February 12, 2018 the employer did not ensure an employee removing plastic pellets from the material hopper on B19 was provided with a safe means of access and egress to the machine frame. While accessing the area adjacent to the barrel by climbing on the exterior of the frame, the employee was exposed to slip and fall hazards. Wilbert, Inc. was previously cited for a violation of an equivalent occupational safety and health standard 1910.24(b), which was contained in OSHA inspection number 1063115, citation number 1, item number 2 and was affirmed as a final order on April 13, 2016, with respect to a workplace located at 635 Southwest St., Bellevue, Ohio.
