29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: Quality Welding & Fabrication, LLC - Elida, Ohio: On or about May 2, 2017 the employer did not ensure that an injury event requiring hospitalization was reported to the local OSHA office within 24 hours. An employee was injured and hospitalized on April 28, 2017 and the injury event was not reported until May 2, 2017.
