29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: 101 North Main Street, Newark, Illinois, west building shop, counterbalance powered industrial trucks - on or about May 4, 2017, modifications that affected trucks' capacities and safe operation were made to fork attachments without the manufacturers' prior written approvals. Holes were drilled in the forks to secure a job-made fifth wheel attachment with pins/bolts for towing dump trailers. Employees were exposed to struck-by and/or crushed-by hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). Louis Dierzen dba Dierzen Sales, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(a)(4), which was contained in OSHA inspection number 1125876, citation number 01, item number 002(a) and was affirmed as a final order on August 29, 2016, with respect to the workplace located at 101 N. Main St., Newark, IL 60541.
