29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth: (a) 1511 Washington Street East: On or about January 10, 2017 and times prior, employees were permitted to perform work in a 3 foot 4 inch by 13 foot 9 inch and up to 5 foot 4 inch deep trench with no safe means of access and egress. Tri-State Pipeline, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.651(c)(2); Means of Egress, which was contained in OSHA inspection number 1029187, citation number 2, item number 1 and was affirmed as a final order on April 27, 2015 with respect to a workplace located at the jobsite in Minford, Ohio.
