29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. The employer failed to ensure that lockout/tagout procedures were utilized and followed on the Telastacker Conveyor when a crew of employees were performing general maintenance on the conveyor exposing employees to unexpected energization. The Johnson County Pipe was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(f)(3)(i), which was contained in OSHA inspection number 953849, citation number 1, item number 2b and was affirmed as a final order on July 15, 2014 with a final abatement date of July 17, 2014, with respect to a workplace located at 800 CR 209, Alvarado, TX.
