Text For Citation: 02 Item/Group: 001A Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a.At the site where sewage pipe was being installed, the bucket of the Komatsu Hydraulic Excavator PC400LC-5 (SN: A70902), that was secured with the Hendrix PC200 Quick Coupler (SN: C5726), was positioned over employees working in the trench. On or about December 6, 2000, an employee in the excavation was fatally injured when the bucket became disengaged from the hitch and fell into the trench. Among other methods, feasible and acceptable abatement methods to correct this include, but are not limited to the following: a.Adopt the CIMA protocol and safe operating procedures which would prohibit operators from positioning the hydraulic arm and its attachments (e.g., the bucket) over personnel. b.Prevent employees from walking or working under any part of the machine or load while the machine is in operation.
