29 CFR 1904.29(b)(2): The employer did not complete an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the OSHA 300 Log: a) 97 Theater Lane, York PA - On or about February 1, 2016, the employer failed to complete an OSHA 301 or equivalent form for a recordable injury which occurred on December 26, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
