29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) On or about 01/06/16, in the Coal Handling building; employees performed service and maintenance work on equipment such as but not limited to, Conveyor Belt 11, Conveyor Belt 15 Elevator and were exposed to crushing and caught on hazards when Hazardous Energy Control Procedures for this equipment were not certified in 2015. ABATEMENT DOCUMENTATION REQUIRED Tonawanda Coke Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147 (c)(6)(ii), which was contained in OSHA inspection number 961334 Citation Number 2, Item Number 1 and was affirmed as a final order on 08/21/14, with respect to a workplace located at 3875 River Road, Tonawanda, NY, 14150.
