29 CFR 1910.119(e)(3): The process hazard analysis did not address the elements of (i) through (vii) of this paragraph: a) For the Flour Bleaching Process - On or about September 30, 2015, the employer's process hazard analysis [PHA] did not to meet the requirements of 29 CFR Part 1910.119(e)(3)(i) through (vii) in that it failed to identify, evaluate, and control the hazards of the process (nitrogen trichloride potential), the degree or range of the safety and health effects of chlorine on employees or the number of employees upon which the failure of controls or the consequences of a deviation might have an impact, or the hazards related to facility siting in that the growing plant had encompassed the existing Flour Bleaching Process, which exposed more employees to small-scale releases and minor equipment failures. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
