29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about July 30, 2015, the employer failed to ensure employees that have to access the side panel of the M-Series Nut Tappers to remove jams utilized lockout/tagout. Telefast Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910147(c)(4)(i), which was contained in OSHA inspection number 990085, citation number 01, item number 001 and was affirmed as a final order on October 30, 2014, with respect to a workplace located at 777 West Bagley Road, Berea, Ohio 44017.
