Text For Citation: 02 Item/Group: 001A Hazard: FALLING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls: (a) On or about August 14, 1994, the American Hoist, S/N: H3430, located at the south side of chimney number 1 at the Tennessee Valley Authority's Cumberland City power Genrating Plant was overloaded, exposing employees to the hazard of falls. Among other methods, one feasible and acceptable abatement method to correct his hazard is to follow established weight limits on the hoisting system. (b) On or about August 14, 1994, employees were required to work on a suspended scaffold located at chimney number 1 at the Tennessee Valley Authority's Cumberland City Power Generation Plant without the protection of life lines. Among other methods, one feasible and acceptable abatement method to correct this hazard is to install life lines anchored to overhead structural members capable of supporting 5400 pounds. (c) On or about August 14, 1994, employees were required to access the cathead located at chimney number 1 at the Tennessee Valley Authority's Cumberland City Power Generation Plant without the use of fall protection. Among other methods, one feasible and acceptable abatement method to correct his hazard is to install a platform and ladder with capabilities for tieing off at the underside of the cathead which permit the employees to safely exit the cage onto the platform while tied off. Another method would be to use a Boatswains Chair attached to the material hoisting cable as outlined in Variation #2 of Union Boiler Grant of Variance.
