29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides and railings were not provided with a toe-board where required: The employer did not protect each employee from fall hazards by use of standard railing or other protective devices. Fall hazards include, but are not limited to, the following: a. On or about 05/05/2015, the portable platform with trumpet base, exposed employees to a fall hazard of approximately 10 feet. b. On or about 05/06/2015, the pump house work platform of bag house four and bag house five, was unguarded. Employees were exposed to a fall hazard of 9 feet. c. On or about 05/21/2015, employees who access the tops of the tundish cars, were exposed to a fall hazard of 9 feet. d. On or about 05/22/2015, the west-end of the shot blaster work platform, located in the IBBI department, had a 6 to 9 inch gap at the edge of the platform. Employees were exposed to struck by hazards from tools fall from the platform above. e. On or about 07/16/2015, the cranes numbers 2;22;14;15;17;19;31;29; and 27 did not have eye bolts or anchor points for tying off. Employees are exposed to a fall of approximately 25 feet. f. On or about 07/17/2015, the landing for crane number 159 located in the Roll shop was not installed. Although the company provided JLGs for use, the lift did not allow access to crane. Employees climbed over end truck of an additional crane, exposing employees to a fall of approximately 25 feet. g. Crane #30, located in the run-out area of the Cast department, did not have end railing on the north-end of the truck exposing employees who access the area to a fall hazard of approximately 25 feet. h. On or about 05/19/2015, employees stood on the HRME transfer cars to stamp the heat number on the ends of the steel bar, were exposed to falls of approximately 15 feet. The TimkenSteel Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.23(c)(1), which was contained in OSHA inspection number 462358, citation number 2, item number 1, and was affirmed as a final order on July 1, 2013 with respect to a workplace located at 2401 Gambrinus Avenue, SW, Canton, OH 44706.
