29 CFR 1926.651(k)(2): Where the competent person finds evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety: a) 211 South Grove Street: On or about March 20, 2015 and at times prior, employees were exposed to an excavation cave in while installing a new four inch sewer line in an unprotected, undercut excavation of a Type C soil. b) 1325 River Street: On or about March 31, 2015 and at times prior, employees were exposed to an excavation cave in while installing a new sewer line in an unprotected excavation of a Type C soil.
