On or about March 18, 2015, in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during setup of the Mori Seiki SL-25 CNC lathe. b) On or about March 3, 2015, in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during setup of the Mori Seiki ZL-45 CNC lathe. c) On or about March 17, 2015, in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during setup of the Mori Seiki SL-25 CNC lathe. d) On or about the week of March 16, 2015, in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during setup of the Mori Seiki SL-35A CNC lathe. e) On or about the week of March 16, 2015, in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during setup of the Mori Seiki SL-35B CNC lathe. f) On machine operator during setup of the Landis 2.0 Double Long Bolt Threader. g) On or about March 18, 2015, in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during setup of the Landis 2.5 Double Short Bolt Threader. h) On or about March 18, 2015, in the machine shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during setup of the Jones/ Lamson 7A Turret Lathe. i) On or about March 24, 2015, in the Dyson Forge Threading (DFT) Division located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during setup of the Manual Four Spindle Upright Tapper. j) On or about March 24, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during greasing of the Tesker Thread Roller number three. k) On or about March 24, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during the greasing of the thrust bearings on the Tesker Thread Roller number six. l) On or about March 3, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator, while cleaning out the oil tray beneath the drive motors and shafts, of the Tesker Thread Roller number six. m) On or about March 24, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator, while cleaning out the oil tray beneath the drive motors and shafts, of the Tesker Thread Roller number one. n) On or about the week of March 24, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator while changing out the dies on the Tesker Thread Roller number one. o) On or about March 18, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator while, cleaning out the oil tray beneath the drive motors and shafts, of the Tesker Thread Roller number three. p) On or about March 10, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator while, cleaning out the oil tray beneath the drive motors and shafts, of the Tesker Thread Roller number six. q) On or about March 18, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during the greasing of the thrust bearings on the Tesker Thread Roller number three. r) On or about March 10, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator during the greasing of the thrust bearings on the Tesker Thread Roller number six. s) On or about April 9, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not utilized by the saw operator during a blade change on the Horizontal Cantilever Band Saw (end). t) On or about April 9, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not utilized by the saw operator during a blade change on the Horizontal Cantilever Band Saw (middle). u) On or about April 8, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the saw operator during a blade change on the Spartan Super saw. v) On or about February 26, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator while, cleaning out the oil tray beneath the drive motors and shafts, of the Tesker Thread Roller number three. w) On or about the week of February 26, 2015, in the Volume Rods Department located at 53 Freedom Rd., Painesville, Ohio 44077, hazardous energy control procedures(lockout/tagout) were not developed, documented, or utilized by the machine operator while, cleaning out the oil tray beneath the drive motors and shafts, of the Tesker Thread Roller number six.
