Located at 391 Varnum Ave. Lowell, MA: On or about 11/30/14, the employer did not record the following workplace injury and illness on the OSHA 300 log for calendar year 2014; a) On or about 11/30/14, a registered nurse working on the adolescent unit was punched in the face and knocked out unconscious by an aggressive patient, which resulted in approximately 2 days away from work.
