29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. a) At the north west section of the excavation, on October 8, 2014, the employer did not ensure the spoil pile, that measured three feet in height, was placed at least two feet from the edge of the excavation, that measured five feet in depth, where an employee was performing preparation work for the installation of force and reclaim sewer lines, exposing the employee to the hazards associated with death, broken bones, lacerations, and bruises.
