29 CFR 1910.119(e)(3)(v): The process hazard analysis did not adequately address facility siting. The employer does not ensure the process hazard analysis addressed facility siting adequately. a. The violation occurred on or about October 7, 2014 and times prior thereto in the Okarche Gas Plant where the employer exposed employees to fire and explosion hazards when it failed to ensure the process hazard analysis adequately addressed the siting of the control room and other personnel occupied buildings in the facility. DCP Midstream LP was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.119 (e)(3)(v) which was contained in OSHA inspection number 910522, citation number 01 item number 01 and was affirmed as a final order on November 25, 2013 with respect to a workplace located at 594 South State Highway 163, Ozona, Texas 76943. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date the employer must submit documentation showing it is in compliance with the standard, including describing the steps it is taking to ensure the process hazard analysis addressed facility siting adequately.
