29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: At the inspection site, in the spray booths, airborne exposures were not evaluated for employees who were required to wear half-facepiece respirators with OV/Acid Gas cartridges while spray finishing gas separators with urethane coatings mixed with activators/hardeners containing isocyanates including, but not limited to, Coastline Industrial Coatings Polyurethane Catalyst and Sherwin-Williams Hi-Solids Polyurethane Activator (Part T), Acrolon 218 HS Acrylic Polyurethane (Part B) Hardener, and Sherthane 2K Urethane (Part B), Hardener.
