29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a. On or about July 30, 2014, an employee performing tree trimming operations form the basket of an 1986, S1800 International bucket truck was not wearing fall protection, exposing himself to a 40-foot fall hazard. The Flyway Access, LLC. dba Conrad Tree Surgeons, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.67(c)(2)(v), which was contained in OSHA inspection number 315477000, citation number 1, item number 2 and was affirmed as a final order on July 6, 2011, with respect to a workplace located at 2811-27th Avenue, Ocala, Florida 34432.
