29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a)On or about 7/24/14 at the fatality trench, the employer failed to ensure there was no more than 25 feet distance to a means of egress in that the only means of egress was at the end of the 75 foot long trench. b)On or about 7/22/14 at bunker W1809, the employer failed to ensure there was no more than 25 feet distance to a means of egress in that the only means of egress was at the end of the 83 foot long trench. c) On or about 7/20/14 at bunker W1809, the employer failed to ensure there was no more than 25 feet distance to a means of egress in that the only means of egress was at the end of the 62 foot long trench.
