29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about 9/26/11 during the installation of a gas main, the employer did not ensure employees were provided a safe means of egress while working in an excavation that measured 4.7 to 7 feet in depth, 25 feet in length, and 10 to 11 feet in width. 1127 CONSTRUCTION INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD CFR 1926.651(C)(2), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312965833, CITATION NUMBER 01, ITEM NUMBER 01 AND WAS AFFIRMED AS A FINAL ORDER ON 03/24/09, WITH RESPECT TO A WORKPLACE LOCATED AT HINES HILL & RTE 8 CUYAHOGA FALLS, OHIO 44223.
