OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and electrical hazards At the scrap/crop sheer, employees operated a defective Abell-Howe Crane, Serial Number 68-424, Model number type J-904r with an electric chain hoist. Defects were such as but not limited to damage safety latch, missing covers on the motor, leaking motor, damaged electrical cord to the hoist, unused opening on electrical boxes, and no max load on the crane. Among other methods, several feasible and acceptable abatement methods to correct these hazards are to: 1. Conduct Inspections on hoist for missing or defective equipment in accordance with ASME B30.16, dated 2012, Paragraph 16-2.1.4 (Periodic inspection), and Paragraph 16-2.1.3 ( frequent inspections), Overhead Hoists. 2. Repair or replace the safety latches and ensure they meet the requirement of the for Manufacturer Owner's Manual Electric Chain Hoist Para 5.3, and ASME B30.16.-2012, Paragraph 16.2, Overhead Hoists. 3. Ensure that installation and associated wiring of electrical components is in compliance with ASME/NFPA 70 as per ASME B30-16 Para 16-1.3.4 Power Connections 4 Conduct Inspections on hoist for missing or defective equipment in accordance with Manufacturer Owner's Manual for Electric Chain Hoist, Para 5.3, Frequent Inspection in accordance with table 5-1; Para 5.4 Periodic Inspection in accordance with table 5-2; Para 5.6, Inspection records must be maintained for the hoist periodic intervals per Section 5.2.4, and available to personnel involved in inspections. Ensure that installation and associate wiring of electrical components is in compliance with ASME/NFPA 70 as per warning on Page 5.
