29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c) and had not complied with 652(b): On or about June 10, 2014, at an excavation at the intersection of Bonaventure Blvd. and San Michelle Drive, in Weston, Florida, the employer failed to provide cave-in protection for employees working inside an excavation that was approximately 12-feet deep.
