29 CFR 1904.39(a): An oral report of a death of any employee from a work-related incident or the inpatient hospitalization of three or more employees as a result of a work-related incident was not made within eight (8) hours after the occurrence to the nearest Area Office of the Occupational Safety and Health Administration (OSHA) or to the OSHA toll-free central telephone number (1-800-321-OSHA): The employer did not make an oral report to OSHA within eight hours after the death of an employee from a work-related incident that occurred on June 2, 2014 near Blue Heron Pond located at 2102 North Trade Days Boulevard, Canton, Texas 75103.
