Rabe Environmental Systems, Inc., 2455 Robinson Road West, Erie, PA 16506, Application Bond Area, on or about April 30, 2014 and times prior thereto: The grounding prong on the DeWalt DWM120 5-inch Deep Cut Variable Speed Band Saw (S/N: 078401) was removed. The band saw was available for use, and was not otherwise tagged out of service, removed from the site or rendered inoperable.
