29 CFR 1915.73(c): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches. a.) Colonna's Shipyard, Inc., Path Finder Vessel: On or about 05/13/14, an employee was observed working in the area of three manholes at the bow of the vessel that were not guarded to prevent the employee from falling 5'10" to the next level. Colonna's Shipyard, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1915.73(b), which was contained in OSHA inspection number 309734663, citation number 1, item number 5 and was affirmed as a final order on 9/28/10, with respect to a workplace located at 400 East Indian River Road, Norfolk, VA 23523. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
