29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely: Employees were involved with an accident regarding dropped stacks on or about May 18th , and/or 19th, 2011, and the employer did not conduct an evaluation of the effectiveness of the retraining provided to the affected employees.
