29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about April 16, 2014, at the above addressed site, an employee was exposed to a fall hazard while trimming fronds from the bucket of a truck mounted aerial lift approximately 20 feet above a lower level and not being protected from falling to that lower level. CARLTON'S TREE SERVICE, INC. WAS PREVIOSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.67(c)(2)(v) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 723261, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS FINAL ORDER ON 12/14/2012, WITH RESPECT TO A WORKPLACE LOCATED AT 2409 N. OCEAN BOULEVARD IN GULF STREAM, FLORIDA.
