29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. a) Roll Former facility - On or about April 8, 2014, during calendar year 2011, employees sustained injuries involving lost work time or restriction and the employer had not recorded the injury on the OSHA Form 300 or equivalent. b) Roll Former facility - On or about April 8, 2014, during calendar year 2012, employees sustained injuries involving lost work time or restriction and the employer had not recorded the injury on the OSHA Form 300 or equivalent. c) Roll Former facility - On or about April 8, 2014, during calendar year 2013, employees sustained injuries involving lost work time or restriction and the employer had not recorded the injury on the OSHA Form 300 or equivalent. d) Roll Former facility - On or about September 11, 2014, the employer had not recorded a fatal accident that occurred on April 8, 2014 on the OSHA Form 300 or equivalent for the calendar year 2014.
