29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness had occurred: On or about March 20, 2014 and at times prior thereto, at the job site, the employer failed to enter a work-related amputation on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days. An employee left ring and middle fingers were amputated while trying to clear a jam from the die of a mechanical press. After compliance officer requested the OSHA 300 Log and 301 Incident Report during the OSHA inspection, the employer input the injury on the OSHA 300 Log.
